Regional home health care provider Almost Family said it has acquired Jacksonville, Fla.-based Ingenios Health Co. for $13.3 million in cash and stock. The company said that Ingenios allows for a strategic entry into the growing health assessment market, as well as the Nurse Practitioner business. Ingenios operates in seven states and the District of Columbia and conducted 10,000 in-home assessments in 2014.

“By using nurse practitioners to conduct comprehensive health assessments in patients' homes and physician practices, we believe Ingenios will provide a foundational basis for improved care planning and delivery with meaningful cost savings,” said William B. Yarmuth, Almost Family chairman and CEO. “We look forward to developing the Ingenios model in markets in which we currently provide home health and ACO services as well as in new markets we can develop together.”

In 2013, the company acquired a controlling interest in Imperium Health Management, a development-stage company providing strategic health management services to ACOs. 

Based in Louisville, Ky., Almost Family has 250 locations in 15 states with more than $500 million in annual revenues.
